Just a quick update, for the online survey monkey questionnaire I also asked Age and Gender questions, as I felt that we needed to make sure we were communicating well with our target audience. However, these questions weren't really appropriate in the paper form, as I knew who had filled them in, and their ages were all in the same age band.
